TRANSCRIPT REQUEST

Need a transcript for grad school or employment applications? We’re happy to work with you to get one. Read the following before requesting:

Request a transcript at least 3 weeks before you need it – ICADS is unable to expedite transcript requests.

Your ICADS credits most likely show up on your home institution’s transcript; please check with your university to see if your ICADS credits are reflected on your transcript before requesting an ICADS transcript. If you find that you still need an ICADS transcript, please fill out and submit the form below.

There is a charge of $3 for each transcript. Please send a U.S. check made out to ICADS to:
